Please complete the information on the following forms as thoroughly as possible to help us ensure verey good care. We will be happy to assist you on items that you are unsure how to answer

C. Referral form primary care physician (if requiered by insurance)
D. Photo ID
E. Complete and accurate list of your medications
In addition, you will need to be prepared to make a payment or co-payment as set per your insurance carrier.
